Medical documentation includes:
- Medical certificate to travel abroad: it certifies the applicant’s state of health and his suitability to travel to any foreign country.
- Medical report: it is the document issued by the doctor responsible for a patient, which indicates the medical aspects of a specific healthcare process, in particular, the disorder suffered by the patient, the diagnosis and prescribed treatment, as well as its the possible consequences.
- Diagnostic tests: it contains detailed information on any study that is performed on the patient, such as tests (blood, urine), ultrasound, cytology, X-ray, MRI, allergy, stress, ophthalmological tests, etc.
- Medical history: it is the compilation of all the medical information related to the health of a patient and the services and medical attention that has been provided throughout his life.
